4. Quote (What one?)
5. Colors (Any specific?)
6. Music box (OPTIONAL. What song? Autoplayed or not?)
7. CSS Background (OPTIONAL. Need a link to the background.)

1. Do not take off credit. I work hard.
2. Don't rush me, I take my time so your layout looks nice.
3. Don't request to me if you're only planning to use the layout for a few days.
4. Do not try to fix your textboxes in the layout, it will mess up everything, and I WILL NOT code it for you again.
5. I would gladly apperciate it if you don't skip any of the requesting form.
6. I don't make layouts for known "jockers" or known "bitches."
